I am writing to make a Freedom of Information request to ask for the information your Health Board has recorded in regards of community pharmacy closures. I am looking for information reported and recorded about all closures from Thursday 01st September 2022 to Saturday 31st December 2022 inclusive.
1. The precise information I am requesting is,
- Contractor Code
- Name, address, and postcode of pharmacy
- Date of closure
- The precise times of closure, and if more than one closure happens in one day, please report each issue as separate.
- Any reason given for the closure by the contractor, if none was recorded please answer n/a or none.
2. I am also looking for both types of closures, those which appear to have been unplanned, and those which were agreed in advance by your health board. If the closure was agreed to and planned in advance can you please state ‘agreed’ or ‘planned’ under ‘reason for closure’, if it was unplanned please give the reason if any stated by the contractor.
I would be most grateful if your answers could be supplied on an Excel spreadsheet.
