I would be grateful if you could send me the following information in regard to Datix incidents:

1. The exact number of daily incident reports in the health board for each of the following years: (a) 2018/19, (b) 2019/20, (c) 2020/21 (d) 2021/22 and (e) 2023 to date

2. Please provide this data broken down by hospital, and by whether these are so-called ‘red incidents’ - or broken down by severity where possible.

3. The exact number of duty of candour investigations in the health board as a whole.

I would prefer all of this information in a digital format. 

Under FOISA s 12(1), NHS Shetland is not obliged to comply with a request if the estimated cost of doing so exceeds £600.[1] Having reviewed your request, NHS Shetland has determined that the cost of compliance would exceed £600 and so is unable to provide the information requested; we have provided a breakdown of the estimated costs below:

The Datix system is unable to run and categorise the data, as requested.  This would need to be carried out manually.

There are over 3060 reports and an approximate calculation would be 5 mins x 3060 = 255 hours to complete. For information, Staff costs are capped at £15.00 per hour.
