I'm looking for the following information through FOI legislation:
1. Confirmation in the form of a list of every type/category of incident that can be recorded on the datix system. In other words, a record of what type of incidents can potentially be recorded on the datix system by a member of staff.
Any untoward event or near miss can be reported via the organisation’s Adverse Events reporting system, Datix. Datix contains a broad range of categories which enable all reports to be categorised depending upon the issue reported.
2. The complete record of incidents that were recorded on datix system in the year 2022 (January to December)
Please provide a breakdown by type/category of incident and the number of times each category/type of incident occurred. If the outcome of the incidents are recorded then please also include them.
Also, provide a breakdown by hospital.
In 2022 there were 663 incidents that have been reported via the Datix system and these have been categorised as shown on the next page.

These 663 incidents cover all issues reported across health care services in both Primary and Secondary care. A search of the Datix system identifies 395 incidents reported from the Gilbert Bain Hospital. The Gilbert Bain Hospital is the only acute hospital in Shetland.
With regards to the outcome of each individual incident, under FOISA s 12(1), NHS Shetland is not obliged to comply with a request if the estimated cost of doing so exceeds £600. Having reviewed your request, NHS Shetland has determined that the cost of compliance would exceed £600 and so is unable to provide the information requested; we have provided a breakdown of the estimated costs below:
Reporting on the outcome of each individual incident would require a manual review of all 663 incidents we estimate it would take at least 5 minutes per record.
5*663=3315 minutes or 55.25 hours
Staff costs are capped at £15.00 per hour therefore where it would take longer than 40 hours this would exceed the £600 cap.
NHS Shetland may be able to provide a response to a new, narrower request for which the cost of compliance would not exceed the cost threshold. Please let us know if you would like to submit such a request.
