I am looking for information regarding GP practices in your health board.
As such, can I request the following, broken down by the following years 2019, 2020, 2021, 2022 and 2023 (to date):
1. The number GP practices in your health board that have closed their practice lists.
2. A list of all GP practices in your health board that have restricted their practice lists.
