I am looking for information on specific Risk Assessment Tools used by Mental Health Services, Social Work Services and CAMHS services across Scotland.
- What tools are used by mental health services in your health board area;
Risk assessment Matrix used in emergency presentations (amended by the team from Avon & Wiltshire Mental Health Partnership NHS Trust, 2002, Emergency and acute medical care in over 16’s: service delivery and organisation, NICE Guidelines 2018, Mental Health Act, 2003, Stonewall, 2018, Mental Health Risk Assessment Tool)
Risk assessment profile used during routine assessments (developed by Imosphere Ltd. – company behind Care Partner software).
- Are your Mental Health Services using validated tools or locally developed or amended tools;

- Same questions in relation to CAMHS services;
In CAMHS we use an initial brief risk assessment (amended from the tier 4 North of Scotland risk assessment) – this covers self-harm/ suicidal thoughts, neglect, violence and exploitation.
If there is evidence of risk of suicide we then use the Formulation Based Risk Assessment – within this we use the suicide assessment and treatment pathway amended from NHS Lanarkshire.

- Are there statistics available to show what, if any, formal risk training has been offered in your area?
We do not have statistics to show formal training but a number of our clinicians have completed formal training on the formulation based suicide risk assessment.
