NHS Shetland is in an initial test phase for H@H. During this phase, we are able to provide care for 2 H@H beds, with the hope that this can be expanded next year to a maximum of 4 H@H beds.

Trak care, which is the Hospital based system, but also use GP electronic systems (EMIS)
- Suppliers/Providers supporting the service.
There are many services that could be involved. Consultant Physicians/Geriatricians from Secondary Care, Urgent and Unscheduled care lead, Advanced Nurse Practitioner, District Nursing services, Allied Health professionals, Pharmacy, Intermediate care team.
