Community pharmacy Closures - FOI Response Register

I am looking for information reported and recorded about all Community Pharmacy closures from Sunday 1st January 2023 to Sunday 31st December 2023 inclusive.

1. The precise information I am requesting is,

Contractor Code

 Name, address, and postcode of pharmacy

 Date of closure

 The precise times of closure, and if more than one closure happens in one day, please report each closure as separate.              

Any reason given for the closure by the contractor, if none was recorded please answer n/a or none.

I am also looking for both types of closures, those which appear to have been unplanned, and those which were agreed in advance by your health board. If the closure was agreed to and planned in advance can you please state ‘agreed’ or ‘planned’ under ‘reason for closure’, if it was unplanned please give the reason if any stated by the contractor.
