NHS Shetland has 1 maternity unit situated in the Gilbert Bain Hospital. Until September 2020 the model was a Midwifery Unit with input from GPs and Emergency Caesareans being performed by surgeons. High risk women were delivered on the mainland. In September 2020 the model changed to being a Midwifery Unit with Consultant Obstetricians providing care for those who require Obstetrician input. High risk women continue to be delivered on the mainland.

No units were closed down, the model changed to provide safer care closer to home where able.
