1. A copy of the latest pieces of correspondence received by the health board concerning under-staffing or dangerous staffing levels.

Specifically the latest correspondence produced in the following formats:

(i) an email written by a staff member (please include any reply from health board or management)

(ii) a letter written by a staff member (please include any reply from health board or management)

(iii) an account of an event recorded on the Datix system.

To be clear, I am requesting a copy of any material that has been written or produced by a staff member (Doctor, Nurse, Midwife) that expresses concerns about staffing, appropriate staffing or a danger or hazard related to staffing.
