Hate Incidents - FOI Response Register

1. Whether the board has any systems on which it records “hate incidents” by or against staff and/or patients, or otherwise holds records on such incidents.

Our adverse event system Datix does not have any specific categories for the term 'Hate'.  Any such incidents would be recorded under the category “Disruptive, violent aggressive behaviour”. However we can search the incident notes for use of the word “hate”.

2. If so, any guidance relating to the use of those systems,  including how such incidents are defined and any information held on the number of such incidents over the most recent 24 months for which such information is available.
