Please provide the following information covering the period from 2018 to present:

- The number of recorded incidents, near misses, attempted suicides, or suicides involving the use of adjustable beds or bed mechanisms as a method of self-harm or suicide in these units.
- Copies of any risk assessments, safety reviews, or policy documents relating to:
o The use of adjustable beds in mental health wards
o Measures taken to mitigate ligature risks associated with beds or bed equipment
- Details of any Serious Adverse Event Reviews (SAERs) or equivalent investigations conducted related to suicide attempts or deaths involving adjustable beds in your mental health wards, including any summary findings or actions taken (redacted to protect patient confidentiality).
NHS Shetland does not have any mental health inpatient beds.
