Please provide the following information for the time period between 1st January 2019 (or the date of inception for your trust) and 5th May 2025, inclusive.

1. Please send me a copy of any documents outlining guidelines or codes of conduct regarding mental health care plans which have been used or disseminated within the Trust.

3. Please send me copies of all Serious Incident Review documents which relate to the treatment of mental health patients within your trust.

In accordance with FOISA s 16(1), NHS Shetland confirms that it holds the information requested but that it is exempt from disclosure under FOISA s 38(1)(b) as read with s (38)(2A) as the information constitutes personal data and disclosure would breach the confidentiality of the data subject(s). The reasons for this decision are that, due to the nature of these events and the small sample size identified, within an overall small geographical population, NHS Shetland believes providing copies of serious incident reviews, will result in potentially patient identifiable information being released. Therefore, reports are not provided but are held in the Datix system.
