Commissioning of Complex Community Care:
- Does your Health Board commission complex care and support in the community for individuals with significant clinical needs, such as those requiring a tracheostomy?
Yes, we will provide complex care for patients within the community following appropriate levels of training and competence (depending on the circumstances)
- If so, please provide details of the types of services commissioned, including whether these are delivered directly by the NHS, through Health and Social Care Partnerships (HSCPs), local authorities, or via third-party providers.
Responsible Teams and Contacts: This would be dependent on the situation, if it was in relation to a child, then that would be the responsibility of the Children’s services, if it was an adult then would be managed by the community nursing team. We would also provide complex care in the community through our Hospital at Home service.
