Adult Social Care - FOI Response Register

I would be most grateful if you would provide me, under the Freedom of Information Act, the details below.

1. An organisational structure chart of the Trust, specifically focussing on people responsible for Adult Social Care and Nursing Home placements.
2. Details of organisational roles involved in discharge, locating and agreeing funding for residential, nursing and other placements, as well as work email addresses and telephone numbers. Job roles would include the following or equivalent within the organisation:

-           Discharge Coordinators

-           Patient Flow Team

-           Hospital Social Workers

-           Discharge Administrators

-           Placement Officers

-           Referrals Team

3. The number of placements in Nursing Care funded by the Trust with costs exceeding £2,500 per week/£130,000 per annum.
4. The number of nursing care placements joint funded with Local Authorities
5. Details of any frameworks or similar procurement arrangements operated by the Local Authority in relation to Adult Nursing and Residential placements.

There are no Nursing Homes in Shetland.
